Les Paul Custom 3 pickups Wiring

Hi, any wiring experts able to tell why the middle pick up is not working? I am trying to do an independant middle pickkup.


Except that i am using a tone pot as a volume pot for it. I suspect this is the issue, unless its the toggle switch. The neck, brigde and combination(middle position) of toggle switch is working perfectly fine.


The default toggle switch is a 6 lugged one, quite different from most les paul switches i know of.
 
Is the output of the middle pickup connected directly to the "hot" output of the jack? The "hot" our from the three way switch and the "hot" output of the middle pickup should be both connected to the output jack. Unless there are any other broken joints in the wiring. The output jack retains itself as a mono jack. From what I see, the 3 way switch operates as the usual 3 way since the parallel input to it is not linked up(the unused leaf). If you do use it, it'll be a parallel operation of the middle pickup with the bridge pickup. Hope this helps.
